Welcome to Class: Forms

There are several forms you will need to complete and return to your school (SCF, PSC, or TCC):

• ASQ Registration form
• Banner Registration form
• FERPA form
• SCF ONLY: Top half only of the CCD Registration form (course name is Lean Bronze Certification; CRN is not yet available)

Please complete the forms right away, and scan and send them to your respective college, along with a copy of your drivers license. For those of you who have taken an ETAM-funded course and have already had your ID verified, you will not have to do it again.
